The confidential estate planning worksheet is the document you complete and then either scan/email to me, or simply FAX. This worksheet asks you to complete what type of trust you want, personal information, such as name, address, phone number, marital status, etc. It will also ask you for the names of all your children, who you want to appoint as the successor trustees, executors under your will, and agents under your power of attorney and advanced healthcare directive.

The form is annotated to give you a brief description of the information requested by section. The information requested will become quite clear after you review the On Line Trust Seminar, or after our first, No Obligation, trust appointment.

Once I receive the worksheet I will call you to go over the information with you, then we can set an appointment to sign all the documents.
